This is the excellent Hawaii map by Griffon, as I cannot find his conatct details, I will just credit him perfusely for this map.

I flew around Honolulu with next to zero lag. It's the most detailed portion of the map, the rest just being a colored stock Hawaii map. The worst shaking I had was on the big island and there were only a few simple elevation grids there. That being said, my mac version will have planes shaking their tailpipes at me when the openGL PC version will be rock steady... stupid mac version...

Is it possible that the shaking comes from distance from the 0,0,0 point of the .fld?

I wouldn't be too surprised if that is (part of) the problem. Maybe that would have something to do with floating point number inaccuracy then. This behavior is common among many programs/games. (Minecraft Far Lands anyone?)

From what I've noticed on both this map and on my Philippine Islands, the closer I am to an elevation grid mountain or the farther away from 0,0,0 the worse the shaking gets.

I've noticed some shaking on other maps, but it seems like it always happens when I'm near something, buildings, ground objects, etc.

The strongest evidence that it's the 0,0,0 factor is more important, is this map. around Honolulu where all the detail is, no shaking, but like i've mentioned, the farther away I go, the more shaking.

Mainly because the closer you are to the object on the ground ether it be an object or an elevation grid the more graphics memory it takes to render them in detail the closer you get.Try setting your graphics quality to a lower setting if you are flying a huge scenery.I.E. ground on auto or low render,Z buffer auto or low.
also on huge scenery's do not use ground textures or runway light textures to boost the FPSs just a bit.
